Player Evaluation Request Form

At Central Florida United Soccer Academy (CFU), our player evaluations are designed to recognize each athlete's unique abilities and potential. Our philosophy emphasizes a holistic approach, focusing not only on technical skills but also on decision-making, teamwork, and overall growth. Evaluations are conducted by licensed coaches in a supportive and constructive environment to ensure every player receives valuable feedback. We are happy to announce our player evaluation dates/times for our first season - kicking off Fall 2025!

Please be sure to indicate which sessions you plan to attend. All players are encouraged to attend all possible sessions to ensure a proper evaluation.

CFU Soccer Waiver of Liability and Release Form (Required)

I, the undersigned, in consideration for my and/or my child’s voluntary participation in Player Evaluations, do hereby willfully acknowledge and attest to my understanding and agreement that: Recognizing the possibility of physical injury associated with soccer and in consideration for Central Florida United Soccer Academy LLC DBA CFU Soccer and its affiliates accepting the registrant for its soccer programs and activities (the “Programs”), I hereby release, discharge and/or otherwise indemnify Central Florida United Soccer Academy LLC DBA CFU Soccer, its affiliated organizations and sponsors, their employees and associated personnel, including the owner of fields and facilities utilized for the Programs against any claim by or on behalf of the registrant as a result of the registrant’s participation in the Programs and/or being transported to or from the same, which transportation I hereby authorize. My son/daughter has received a physical examination by a physician and has been found physically capable of participating in the Programs. I hereby give my consent to have an athletic trainer and/or doctor of medicine or dentistry provide my son/daughter with medical assistance and/or treatment and agree to be responsible financially for the reasonable cost of each assistance and/or treatment.

I have read and understand this consent form, and I volunteer to participate. As a parent or guardian, I have read and understand this consent form and I give permission for my child to participate.

Guided by professionally licensed coaches who fully embrace the responsibility of coaching and player development, we aim to inspire athletes to achieve their full potential and cultivate a lifelong love for the game. Our Vision is to meet players at their current level and provide the environment and tools for them to grow and develop.
